W-4 Form:
Please use your name as written on your current social security
card.
I-9 Form:
When hired for a new job, employees are required to prove that they
are legally entitled to work in the United States.
Employers are required to verify the identity and eligibility to
work for all new employees. An Employment Eligibility
Verification Form (I-9 Form) must be completed and kept on file by the employer.
PA Residency Verification for Local Tax
Withholding
PA Residents only:
Please fill out the top portion of this form (resident
location). This will determine in which municipality
your local
taxes will be withheld.
Timesheets:
Keep track of your daily hours on the time sheet. Have the Manager
sign off every week. Please Email (or fax) your time sheet at the
end of
the week/before you leave for the weekend so we'll have it by 9:00
am Monday morning.
Failure to fax or email your time sheet or
no signature from Manager can result in late payment.
Paychecks:
Temporary employees of GSA Financial Staffing are issued biweekly
(every other Wednesday) with paychecks mailed out from our
Blue Bell
office.

drug screen is requested, please download these forms:

PA Drivers DL-503 (PA
Residents Only.Complete
parts C&E)
If a drug screen is
requested we will overnight to you a
MedTox Chain of Custody
Form.
This form cannot be emailed
or downloaded. Contact Marlene directly. Also, testing sites will be provided to you.
Please do not attempt to
test at a site that was not provided to you. Thank you for
your cooperation.
Please fax or email (preferred) the completed W-4, Residency
Verification (if applicable) and I-9 with a copy of two forms of
identification. If a background and/or drug screen is
requested, please also send back the necessary forms. If you have any
